Forms - Mailing Dates and Due Dates

Parent/Camper Information Sheet (Cabin Mate Request), Health Examination Form, Letter to my Counselor Form, Permission to Leave Camp Form will all be mailed out to families in mid to late March.  At that time we will also post these forms on the web-site.  Forms are all due back to YMCA Camp Carson prior to May 21st.

Note: When completing the initial family account information please take the time to complete all the requested information, (parents addresses, phones etc.)  This will enable the system to autofill much of the camper application for each child registered.  Unfortunately if you skip over the initial parent information questions, you will have to manually enter this information on each application.
